Lease Paperwork

Maryland 4-Hers may own or lease their project animals. 4-H Members may not have both owned and leased animals within the same project area during the same 4-H year (which runs January 1 through December 31). Deadline for lease agreements is June 1st annually or prior to your first 4-H event, whichever comes first.

ID Cards (Alpaca, Dog, and Horse)

If you are planning to show or participate in any alpaca, dog, or horse events during the year, you need to register your 4-H project animal with the Frederick 4-H Office by completing a Maryland 4-H identification card. Deadline to file is June 1st of the current year.

AHQA

The site will open in March and remain open through July each year. Completion of this program is required to show your project animal(s) in County and State 4-H Shows. The deadline to complete the training is July 1 of the current year for Frederick County 4-H members. Click here for Program Guidelines and Tips.

Beef, Sheep, and Swine 4-H Club Scholarship - due April 1st to the 4-H Office
Frederick County 4-H Camp Center Scholarship - must be postmarked by March 1st and mailed to the address listed on the application
Darren S. Kennedy Scholarship - application due by April 30th
Maryland 4-H Foundation Scholarship - deadline is typically in June each year
Maryland State Fair Scholarship - deadline is June 15th of each year

	Diamond Clover Forms (Levels 5 and 6) September 1: Deadline for Level 5 and Level 6 Diamond Clover PROPOSALS due to the Frederick County 4-H office. The Frederick County 4-H Educators will review the proposals and schedule a meeting prior to September 15 to review and recommend changes. September 30: Deadline for Diamond Clover Level 6 proposal to be uploaded into Maryland 4-H's Level 6 Proposal Portal. After September 30, The Central Cluster (Frederick, Howard and Montgomery Counties) Committee will review all Level 6 Proposals submitted through the Maryland 4-H Proposal Portal, and will send feedback to 4-Hers in October for recommended changes to be made to their proposal. December 1: All Level 6 Proposals that have been updated are to be resubmitted into the Level 6 Proposal Portal for the Central Cluster Committee and Maryland 4-H's approval. September 1 (of the next year): Level 6 Diamond Clover FINAL Report is due to the Frederick County 4-H office for review. September 30 (of the next year): Level 6 Diamond Clover FINAL Report is due to Maryland 4-H through the Level 6 Report Portal
As needed at least 30 days prior to event/purchase	4-H Club Fundraising Request form Please submit this form for any fundraising events your club plans to hold. Due 2-4 weeks prior to the event. 4-H Club Purchase Request form Please submit this form for a club purchase that is $500.00 or more. Due 1-4 weeks prior to purchase. 4-H Club Facility Reservation/Contract Request form Please submit this form when reserving a facility for 4-H Club meetings or activities.
