Animal Science Forms
Livestock Registration Forms:
- 4-H Livestock Registration - Market & breeding beef, feeder steers, market & breeding goats, market & breeding sheep, dairy cattle, and swine. One form per species.
- 4-H Quail Registration - For game birds only.
- 4-H Poultry Registration - Breeding and market poultry (No game birds).
- 4-H Rabbit Breeding Registration - Breeding rabbits only.
- 4-H Rabbit Market Registration - Market rabbits only.
- County Bred Market Animal Certification - You may list more than one animal of the same species on a form if from the same breeder. (Prints 2 per page)
- Bred & Owned Market Animal Certification - You may list more than one animal of the same species on a form which were bred/owned by you. (Prints 2 per page)
Horse Forms:
- 4-H Horse Registration - List all horses used in your 4-H project.
- MD Performance Animal Lease - Guidelines and lease agreement*
- MD 4-H Statement of Understanding and Release - Submit with your lease agreement*
- Horse Show Registration for Fair - Submit by July 1 to register classes for the horse show held at the Allegany County Fair.
Animal Husbandry and Quality Assurance (AH&QA) Program:
Must be completed by July 1 to exhibit animals at the fair.

Diamond Clover Forms
The Diamond Clover Award Program engages 4-H youth in a variety of projects and activities that will enable them to acquire skills related to leadership, community service, civic engagement, and advocacy. The award consists of six levels that require a 4-H member to plan and accomplish a broad range of age-appropriate goals with supportive volunteers and community members.

4-H Project Records:
- Maryland 4-H Project Record - Submit one for each general project completed. (Do not use for animal projects. Animal project forms are below.)
- Maryland 4-H Summary Record - Submit only one for the current 4-H year.
- Record Book Guide for Allegany County 4-H
- Clover Record Book - For members 4-H age 5 to 7 - Submit one for the current 4-H year.
4-H Animal Project Records:
- 4-H Large Animal Breeding Project Record - Complete one per species for Breeding Animals (Beef Cattle, Sheep, Swine, Meat Goat, and Dairy Goat).
- 4-H Market Livestock Project Record - Complete one per species for Market Animals (Beef/Dairy Steer, Market Goat, Market Sheep, and Market Swine).
- 4-H Poultry Project Record - Complete for all poultry.
- 4-H Rabbit and Cavy Project Record - Complete for rabbit (one for market and one for breeding) and cavy.
- 4-H Horse Project Record - Complete for horse project.
- 4-H Dog Project Record - Complete for dog project.
- 4-H Small Pet Project Record - Complete for cat, gerbil, hamster, snake and other small pets.
Sponsorship/Scholarship Forms
Allegany County Sponsorship/Scholarship Opportunities:
- 4-H Allegany County Request Form- Sponsorships are available to current Allegany County members/volunteers for camps, workshops, etc.
- Allegany County 4-H Scholarship Form - First-year scholarship applicants for college/trade schools (current or previous Allegany County 4-H members) - Due June 1 each year.
- Allegany County 4-H Scholarship Renewal Form - Returning scholarship applicants for college/trade schools (current or previous Allegany County 4-H members) - Due June 1 each year.
Maryland 4-H Scholarship Opportunities
- Maryland 4-H Foundation Scholarships - Each year Maryland education scholarships are awarded to current Maryland 4-H members or alumni(ae) who will be enrolled at an accredited college, university, or post high school vocational program in the fall of the year in which they apply.
